All our service users receive an evaluation questionnaire from Asist. It is either sent or handed to the user. The user can send it back using a pre paid envelope. There is no way of identifying the person from the questionnaire unless they identify themselves.
Below is the letter that we include with the questionnaire (printed on our letter heading) followed by an outline of the questionnaire itself.  We would always be happy to send you a real copy if you want to see it in all its glory!

We would like to know what you think about our Advocacy Service.

It is important that we know what you think about us so that we can improve how we work with you and how we support other people.

It would really help us if you could take some time to fill in this questionnaire. If you have any problems, please feel free to ask a friend or someone you trust to help you answer the questions.

There is no way we will know who has answered the questions. We do not ask for your name or the name of your advocate.

All the questions can be answered with a tick or a cross. Feel free to add any comments or suggestions you have about us on the form or on another piece of paper
